Glenview (12 Week Session) Sundays
To secure your spot in class, you must complete the following:
- Complete this form as accurately as possible for both you and the student.
- Each child requires a separate enrollment form.
- $20 Deposit per student on this form.
- Then Submit REMAINING FULL payment BEFORE the 1st day of class via mailed check, quickpay/zelle or venmo.
- Review, sign & return class handbook & waiver form BEFORE 1st day of class.
IMPORTANT DATES
- Informal showcase will take place on the last day of class for the Fall Session.
- Formal Recital is on Sunday, May 3rd afternoon at Guerrin Prep High School
- Dress Rehearsals will be on Saturday, May 2nd from Morning-Afternoon
FULL PAYMENT Can Made in the following ways with Students Full Name & Location & Class Time written in the MEMO:

- Must be sent before classes begin
CLASS HANDBOOK & WAIVER FORMS
- Will be sent via Docusign to review and sign by September 1st.
- It must be returned before classes begin
