Kids and Youth Art Camp: Too Cool for Summer!

MEDICAL AUTHORIZATION AND ACKNOWLEDGEMENT
As parents/guardians of (child's name), I/we do hereby authorize the studio (instructors and staff) to make any and all emergency medical decisions and to authorize and consent to, any and all emergency medical care deemed necessary, to be rendered to the above-named child for his/her care and safety.
The undersigned understands that all reasonable and diligent efforts will be made to contact the undersigned parents/guardians of (child's name) in an effort to obtain consent to all medical treatment unless the delay in such treatment could jeopardize the child.
As the parent/guardian of (child's name) the undersigned takes full responsibility for any financial cost that may be incurred for medical care of the above-named youth. Additionally, the undersigned agrees to indemnify and hold harmless the studio (instructors and staff) from any/all liability occasioned by such medical treatment and decision-making.
The studio (instructors and staff) will not administer prescription medications or over the counter drugs except in the case of emergency, such as an Epinephrine autoinjector.
Parents/guardians must inform the studio of any special circumstances which may affect the child’s ability to participate fully and within the guidelines of acceptable behavior, including, but not limited to, any serious behavioral problems or special circumstances regarding psychological, medical, or physical conditions.

WAIVER OF LIABILITY
I understand that the studio (instructors and staff) are acting only as agents and shall not be responsible or liable for any injury/ accident/ illness, which may occur on site or within the duration of the camp. Please be aware that we must be able to reach you in the case of an emergency. All classes will be conducted with utmost care in a safe learning environment.
Please make arrangements to drop off and pick up your child on time. No child will be released without proper authorization from the parent/ guardian in advance.

We routinely takes photographs and videos at our events, both in studio and at offsite events. I, do hereby release to Creative Spark Studio Inc., its assigns, and representatives the irrevocable right to use my picture, images of my art, photograph or video image in all forms and media and in all manners, including composite or distorted representations, for any and all purposes including advertising in all forms, for unlimited time, including written copy, that may be created in connection therewith. I have read this release and am fully familiar with its contents. I am the parent and/or guardian of the minor named above and have the legal authority to execute the above release.

A $50 cancellation fee applies to all registrations canceled a minimum of fourteen (14) days prior the start date of the session. Cancellations made less than fourteen (14) days before the start of the session forfeit payment and will not be refunded. No refunds are issued for no-shows or withdrawal after camp begins. You may reschedule to a different session ONE time without a cancellation charge if the change is made fourteen (14) or more days prior to the start of the originally scheduled session. The cancellation fee is only waived one (1) time. If you wish to reschedule and there are no sessions available, your current session will be canceled and a $50 cancellation fee will apply. The studio reserves the right to cancel or modify a workshop or class at any time.
