Waivers

Please read our policy

Liability Waiver
I understand that my minor child will be participating in a summer camp program which may include activities such as being on a stage, craft activities, outdoor play, and splash pad play. While these represent minimal hazards and although students will be supervised by camp councilors, I understand that no activity is completely risk free and agree to hold the Hilliard Arts Council, its staff, councilors, and volunteers harmless in the event of injury. I understand that if my child regularly causes a substantive disruption or a safety hazard to themselves or others, including ignoring safety instructions, my child may be asked to leave the activity and I will not receive a refund.

Photo Release
I understand that occasionally, people representing various media outlets or the Hilliard Arts Council itself may take photographs or video of classes, events, or activities. I authorize these media outlets and the Hilliard Arts Council to use, reproduce and/or publish photographs and/or video that may pertain to me—including my image, likeness, and/or voice without compensation and that this material may be used in various publications in print or online, for recruitment material, or other related endeavors.

Permission to Transport
While the Hilliard Arts Council will make every effort to reach parents and emergency contacts, in the event of a serious or life-threatening illness or injury, I authorize the Hilliard Arts Council and any agents thereof to summon an ambulance, at at the discretion of medical first responders, to transport to an appropriate hospital facility. I understand that Hilliard Arts Council, nor any of its staff or agents, will be responsible for any expenses incurred as a result.

Medication Administration
If my student requires emergency aid such as an EpiPen, asthma inhaler, or other emergency devices, I understand that I will need to provide such medication or devices, communicate how and when to administer said emergency treatments in writing to the program director, and so authorize the use of such. I understand that the Hilliard Arts Council is not responsible for loss or damage to any item I send with my child.

Allergy Notification
I understand for my child's safety that I must communicate all allergies or relevant medical information to the program director, in writing. I understand that while safety protocol will be in place to help my child avoid allergies, no indoor or outdoor environment for on- or off-site may be completely guaranteed to be allergy free.

COVID & Flu Notification
I agree not to send my child to camp with flu-like symptoms such as fever, nausea, vomiting, diarrhea, or coughing. I agree that if anyone in the child's immediate household has tested positive for COVID-19, I will notify the camp counselor immediately and await instructions as to whether it's safe to bring my child to camp. I may receive a full or partial refund for a COVID/flu illness depending on how many days of camp will be missed.

I understand that at any time the Hilliard Arts Council may require masks or implement any other safety protocol to avoid the transmission of communicable flus including COVID-19, or to comply with state or local public health guidance. If I do not agree with the safety protocol, such as wearing masks, I understand that I am not entitled to a refund for removing my child from camp.
