CAMP: Summer Mashup 9:30am Week of 8/5

TERMS & CONDITIONS
*We take the health of our campers and staff very seriously, for this reason we will be implementing a variety of measures to remain as safe and healthy as possible. Our precautions are as follows: - All of our camp counselors will have their temperatures checked daily. - Upon entry, all campers will have their temperature taken. If their temperature exceeds 100.4 degrees, they will be asked to stay home. - Campers that show any symptoms of illness will be asked to go home. We also ask that if your child has come into contact with anyone displaying symptoms that they stay home. We will use disposable materials when applicable and all shared spaces and supplies will undergo frequent and rigorous sanitation. - Hand washing and sanitation will be enforced upon entry and prior to lunch and snacks. By checking the box below I acknowledge and agree that, Picasso's Creative Workshop is not responsible for any possible exposure to illness or infectious diseases including but not limited to influenza, MRSA, and COVID-19. While particular safety precautions and personal discipline may reduce the risk of illness, a risk still does exist. As a parent/legal guardian I knowingly and freely assume all such risks, both known and unknown and assume full responsibility for my child's participation. On behalf of myself and my children hereby release and hold harmless Picasso's Creative Workshop and their employees, owners, and other participants with respect to any and all illnesses to the fullest extent permitted by law.
*Your purchase (deposit in applicable cases) is non-refundable within 7days of session. If you are unable to attend for any reason; such as schedule conflict, emergency, illness or weather-related, please notify us by phone at 561-439-4000 or email (picassospottery@gmail.com). If we receive advanced notice, your payment will be held on account and applied towards another like event in the future, or can be used as a store credit.
*Credits do not expire, NO shows, however, will not receive a refund or credit, your payment will be forfeited in the event you do not notify us.
*Due to the nature of kiln-fired arts, slight imperfections can occur. And on rare occasions, kiln fired pieces can crack, shatter or do the unexpected during the firing process. While we take every precaution to ensure that this does not happen, there are some factors that are beyond our control. If a piece cracks or shatters, a credit will be issued along with a ginormous apology because we hate to see bad things happen to your creations.
*GENERAL RELEASE: Pictures and/or videos may be taken during our workshops to use in print and online marketing/advertising.
*AUTHORIZATION FOR EMERGENCY MEDICAL ATTENTION: By accepting the Terms & Conditions, I grant my authorization and consent for Picasso’s Creative Workshop and its staff to administer general first aid treatment for any minor injuries or illnesses experienced by the guest/customer/minor. If the injury or illness is life-threatening or in need of emergency treatment, I authorize Picasso’s Creative Workshop to summon any and all professional emergency personnel to attend, transport, and treat the guest/customer/minor and to issue consent for any X-ray, anesthetic, blood transfusion, medication, or other medical diagnosis, treatment, or hospital care deemed advisable by, and to be rendered under the general supervision of, any licensed physician, surgeon, dentist, hospital, or other medical professional or institution duly licensed to practice in the state in which such treatment is to occur. I agree to assume financial responsibility for all expenses of such care.
*LIABILITY WAIVER: I hereby release, hold harmless, defend and indemnify Picasso’s Creative Workshop and its employees, agents, officers and directors from any and all damages, injuries, claims, and causes of action which may accrue to or be asserted by me or any minor child of mine arising directly or indirectly out of my minor child’s participation in a workshop.
