Medical Authorization and Waiver of Liability

Waiver, Release and Assumption of COVID Risk Form. I hereby give my permission for my child to attend the summer camp at Creative Color Art Studios (CCAS). My child and I are familiar with, and knowingly and voluntarily accept, any and all risks associated with attending summer camp at CCAS. I specifically assume all risks and hazards associated with my child’s participation in the camp including, but not limited to, the risks associated with the novel COVID virus. I understand that although the children and staff will have their temperatures taken upon entering the camp, that precaution is not adequate to prevent the spread of COVID, give the fact that many infected persons are asymptomatic. I understand and voluntarily assume the risk that my child may acquire COVID. I certify that my child is in good health, has no fever, and has no current issues that make it unsafe for my child to participate in the camp, which does not have a medical professional on staff. I will notify CCAS and not send my child to the camp if my child develops a fever or illness or tests positive. To the fullest extent permitted by law, I hereby agree to waive, release, and discharge any and all claims, causes of action, damages, and rights of any kind against CCAS and all of their respective employees, agents, representatives, and volunteers (the “Released Parties”) arising from or relating in any way to any damage, injury, trauma or illness that may occur to my child, me, or my household members—whatever the cause—due to my child’s participation in the camp. This includes, without limitation, any claim arising from the negligence of the Released Parties. I further agree not to sue the Released Parties, and to defend and indemnify the Released Parties for all claims, damages, losses, or expenses, including attorneys’ fees, if a suit is filed concerning an injury, illness, to me, my child, or my household members resulting from participation in the camp.

In order to keep our students with allergies safe, we ask that you NOT pack peanuts or tree nuts in your child's snack or lunch. WE ARE NOT CONSIDERED A NUT-FREE SPACE, but try our best to keep the Studio safe for everyone. If you do not agree and opt to pack nuts, please notify the Studio and we will make arrangements for your child.

Cancellation and Refund Policy

Please understand that our camp sizes are kept small to facilitate learning, resulting in most camp sessions quickly filling. Last minute cancellations keep other kids from enjoying our program. This is an important issue, thank you for your understanding. CANCELLATION POLICY BEFORE CAMP BEGINS on May 28, 2024: Any cancellations seven (7) days prior to the start of the session, the studio will refund tuition less a $75 cancellation fee per session, per student. CANCELLATION POLICY AFTER CAMP BEGINS: Any cancellations seven (7) days prior to the start of the session, the studio will refund tuition less $150 fee per session, per student. If you cancel seven (7) days or less prior to the session start date, you will forfeit your full tuition. SWITCHING SESSIONS: There will be no fee charged to switch sessions before the start of camp on May 28, 2024. After May 28, a $35 fee will be charged provided the studio is notified seven (7) days before the session starts. If you wish to change sessions and there is no availability, you will be refunded your tuition less $150 cancellation provided the studio was notified seven (7) days prior to the start date of the session. COVID MASK POLICY AND CANCELLATIONS: We reserve the right to update our mask requirement policy at any time based on infection rates and CDC recommendations. Parents will be emailed prior to class with any mask requirement changes. Our cancellation policy stands in the event that we add/remove our mask requirement.
