I understand that my monthly billing will continue regardless for days out of school. Students are encouraged to attend evening classes when the after school program is not in session. My billing rates are calculated based on the continuance of the program without starting and stopping the program through out the year. If I choose to stop the program I must give a 30 day notice.

I understand that if I freeze or cancel my membership before the end of the school year I will be responsible for a $75 cancellation fee and I will be required to re-enroll when returning.

I understand that I must contact Champion Sport Karate when my child will not be attending the program. This call must be before 2:30 pm.

I understand that I will be required to sign an Authorization for Emergency Medical Care and an Transportation Release form prior to my child attending.
