A 50% deposit will be collected today to hold your date and applied to the birthday party charges. Deposits are non-refundable if party cancellation is more than 24 hours after booking. Parties can be rescheduled once if notice is provided prior to 48 hours of the original booking date. Rescheduled parties must occur within 60 days of the original party date if a party slot is available, otherwise Summit Play Village will work with you on rescheduling. Parties can only be rescheduled once. If a reschedule is necessary within 48 hours of the party slot, there is a $75 fee for the reschedule.

Unless you have booked additional time, you may NOT enter the Party room until the start of your party. No exceptions. Unless you have booked additional time, you must vacate the Party room within 10 minutes of your party end time. No exceptions. The Party room must be left clean after your party. Failure to vacate the party room on time will incur a $50 fee for every 15 minutes past your scheduled end time. Although rare, a $50 fee may be applied if the room is left in an UNREASONABLY dirty condition beyond normal use. Please note, Summit Play Village Staff will help clean up.

All guests, including Adults, must have a signed waiver. We recommend including the link to your waiver on your invitations or shared via email prior to the party. All guests must have their waiver validated upon entry.

I. GRIP SOCKS /RISK OF INJURY: Grip Socks must be worn at all times. If grip socks are not worn, parents are aware that should incident or injury occur we are not responsible. Parent/Guardian/Responsible Adult listed on this form understands the risk of injury and that Summit Play Village cannot be held responsible for any accidents. Supervision is required at all times. No drop offs.

We are a shoe free facility. Please make your party guests aware of this. We recommend making a note on your party invitations. Anyone who refuses socks and removing of shoes will be denied entry. All guests INCLUDING ADULTS must remove shoes and wear socks. Guests without socks may purchase them at the cost of $2.50 a pair at the register upon entry unless you decide to pay for them(see below).

II. EVENT DESCRIPTION / VENUE ACCESS: The Renter shall have access to and use of the venue during the reservation time for the purpose of hosting the Renter’s event. Owner shall provide access via on site staff member, and other items necessary to the reservation. III. EXPIRATION OF RENTAL PERIOD: Upon the end of the rental period, all trash must be placed in the provided trash cans. The space must be returned to the condition in which it was found by the time the reservation ends. Failure to clean the space, including removal of trash, décor, and food items, may result in an additional cleaning fee. An excessive cleaning fee of $50 will be charged if the space is left significantly dirty. IV. REMOVAL OF BELONGINGS: Renter shall remove all personal property, trash, and other items that were not present in the venue when Renter took control of it. A trash can, and trash bags are provided. Renter will dispose of all décor in trash bags, or take with them.

V. USE OF PREMISES The Leased Premises shall be used only by Renter and Renter’s guests for a birthday or special event and for no other purpose without prior written consent of Owner. Alcoholic beverages are not permitted at any time. Smoking is prohibited inside the building and anywhere on the premises. If smoking is detected, a minimum $500 cleaning fee will be charged. Vaping is prohibited inside the building and on the premises, and any individual found vaping will be required to leave immediately. Red-colored drinks are discouraged due to the risk of staining. Refrigerator and freezer access are not available for event use. VI. DAMAGES Renter is responsible for any damage to the Leased Premises, including but not limited to walls, doors, floors, furniture, fixtures, and equipment, caused by Renter or their guests. Any damage to walls, including holes, peeling paint, adhesive residue, or markings, will result in a $500 damage fee. Additional damage beyond normal wear and tear will be charged at the full cost of repair or replacement. A minimum damage charge of $500 applies, and charges may exceed this amount based on the extent of damage. VII. DISPUTES Any disputes arising under this agreement shall be resolved in the Owner’s local jurisdiction. This agreement may be executed electronically, and electronic acceptance shall be considered legally binding and equivalent to an original signature. By accepting this agreement, both parties agree to all terms and provisions outlined above.
