Art Classes for Home Schoolers

MEDICAL AUTHORIZATION AND ACKNOWLEDGEMENT As parents/guardians of (child's name), I/we do hereby authorize the studio (instructors and staff) to make any and all emergency medical decisions and to authorize and consent to, any and all emergency medical care deemed necessary, to be rendered to the above-named child for his/her care and safety. The undersigned understands that all reasonable and diligent efforts will be made to contact the undersigned parents/guardians of (child's name) in an effort to obtain consent to all medical treatment unless the delay in such treatment could jeopardize the child. As the parent/guardian of (child's name) the undersigned takes full responsibility for any financial cost that may be incurred for medical care of the above-named youth. Additionally, the undersigned agrees to indemnify and hold harmless the studio (instructors and staff) from any/all liability occasioned by such medical treatment and decision-making. The studio (instructors and staff) will not administer prescription medications or over the counter drugs except in the case of emergency, such as an Epinephrine autoinjector. Parents/guardians must inform the studio of any special circumstances which may affect the child’s ability to participate fully and within the guidelines of acceptable behavior, including, but not limited to, any serious behavioral problems or special circumstances regarding psychological, medical, or physical conditions. WAIVER OF LIABILITY I understand that the studio (instructors and staff) are acting only as agents and shall not be responsible or liable for any injury/ accident/ illness, which may occur on-site or within the duration of the camp. Please be aware that we must be able to reach you in case of an emergency. All classes will be conducted with the utmost care in a safe learning environment. Please make arrangements to drop off and pick up your child on time. No child will be released without proper authorization from the parent/ guardian in advance.

*Missed classes can be made up the following month if 48 hr advance notice is provided. *Cancellations made inside 48 hours of event are not eligible for refund. *Students arriving more than 30 minutes late will find it difficult to catchup as they missed the instructional portion of the class. Please plan to be on time. :)

I understand this registration is for 1 month's worth of group art classes meeting every Thursday, except where noted in the description. I fully understand that my child will be expected to attend classes each Thursday of the month.
