Donation Request Form

We are happy to consider supporting your organization's fundraising endeavors through the donation of a gift certificate for a set of free studio fees. Please fill out our form:

What you need to know... 

• In an effort to create consistency and fairness to the large number of organizations that ask for us support, we created this "Donation Request Form" for you to fill out. This helps us stay organized! 
• Please be sure to submit your "Donation Request Form" at least 3 weeks prior to your event date so we have time to consider your request.
• In an effort to fulfill as many requests as possible, Crackpots will only honor 1 request per organization in a year's time.
  
• Your designated contact will be called when your certificate is ready for pick up at Crackpots Studio at 505 Main Street, Longmont or if we are unable to fulfill your request. We do our best to help as many organizations as we can!
• We ask that telephone inquiries regarding the status of your request not be made. You are welcome to e-mail us at donations@ecrackpots.com if we have not gotten back to you within 3 weeks.
• Please understand that we have already pre-selected several organizations and events for monetary contributions and sponsorships. At this time, we are only taking requests for silent auction items or door prizes.
