I do hereby consent and agree that I am responsibly participating in a Cork and Canvas Class at Clay Cafe Avalon. This event is a BYOB and as a result of my participation, I will not hold Clay Cafe Avalon, the owner/staff or the instructor responsible for any expense or liability incurred as a result of my participation in this class, including medical expenses due to any sickness or injury incurred as a result. I represent that I am 19 years of age, have read and understand the foregoing statement, and am competent to execute this agreement. POLICY: Please note all deposits & tickets are non-refundable, but they are transferable should you choose for someone to take your place. Please arrive on time as class begins promptly upon scheduled time. We will contact you via email in the event of a class cancellation. Reminder: We encourage wearing old clothes as we will be using acrylic paint! Aprons are on hand should you need one.
