Thank you for enrolling in our upcoming 2019/2020 season! We are excited to share our art and passion for dance with you and your family!

Hinsdale ​(12 Week Session) Mondays

To secure your spot in class, you must complete the following:

• Complete this form as accurately as possible for both you and the student.
  
• Each child requires a separate enrollment form.
  
• $20 Deposit per student on this form. 
• Then Submit REMAINING FULL payment BEFORE the 1st day of class via mailed check, quickpay/zelle or venmo.
• Review, sign & return class handbook & waiver form BEFORE 1st day of class.

​IMPORTANT DATES

• Informal showcase will take place on the last day of class for the Fall Session.
• Formal Recital is on Sunday, May 3rd afternoon at Guerrin Prep High School 
• Dress Rehearsals will be on Saturday, May 2nd from Morning-Afternoon
