Photo Release Waiver

I understand that my child will not be released to any person(s) NOT previously listed above. My child will NOT be released to any person(s) who seem to be under the influence of drugs or alcohol. I understand that it is MY responsibility to sign in my child in the morning and sign out my child before leaving in the afternoon. Sign in/out sheets will be available at the front desk and a photo ID will be required for pick up. I understand that my child will not be allowed to leave the program with an unauthorized person. I understand that the Art Factory is mandated to report any suspected cases of child abuse or neglect to the appropriate authorities for investigation. I understand that the Art Factory staff and volunteers are not allowed to transport my child(ren) at any time outside our facility and program. If a violation of this policy is discovered, the Art Factory will take immediate disciplinary action.

Please advise if there is anything else we need to be aware of (ie: Person(s) NOT authorized to pickup, any chronic physical problems or pertinent developmental info or any special accomodations needed. If time allows, we may offer face paint or glitter tattoos. All of our products our FDA compliant and safe to use on the skin. If you'd prefer your child opt out of these offerings, please let us know. You may also call or email the office to further discuss any questions or concerns you may have. (optional)

Camp fees are non-refundable. If you are unable to attend, please give us a call and we will do our best to reschedule with you.
