As a participant in the program, I recognize that there are certain risks of physical injury and I agree to assume the full risk of injuries, damages, and/or loss which I may sustain as a result of participating in any and all activities with or associated with such programs. I agree to waive and relinquish all claims I may have as a result of participating in the program against Brickton Art Center, and their officers, agents, servants, and employees. I do hereby release and discharge the Brickton Art Center, and their officers, agents, servants, and employees, from any and all claims from injury, damage, and/or loss which I may have as a result of participating in the program. I further agree to indemnify and hold harmless and defend the Brickton Art Center, and their officers, agents, servant, and employees, from any and all claims resulting from injuries, damages, and losses sustained by me and arising out of, connected with or in any way associated with the activities of this program. I recognize that there are certain I have read and fully understand the Program Details and Waiver Release of all Claims. Waivers MUST be read and agreed by participant(s)' legal guardian. Facsimile copies will be considered as original by Brickton Art Center.

I understand and acknowledge that Brickton Art Center/WIP Theater has a NO nut product requirement for participants and their families. This means that I promise not to provide my child with any food items containing nuts, nut by products or made in contact with nuts for consumption at Brickton or any Brickton functions. Should your child have any other allergy please inform Brickton Art Center at the time of registration. If your child requires any medicine on site (EpiPen or Benadryl) the parent must provide with instructions to Brickton Art Center/WIP Theater.
